Declaration of Application Questions

Hi -

I was wondering if anyone knows if I need to get the "Declaration of Application" translated? Or do I need to check with the Consulate? The Consulate I will need to go to is Los Angeles, don't know if that makes a difference or not.

Thanks In Advance
