Multiple birthdates, how to get Certificato di nascita?

I have several different dates that have come to light related to my Grandfathers birthday. This makes me uneasy about ever being able to find his Italian certificate, if I mail the Commune can I give them a couple of dates?

Does anyone know where I might find a sample affidavit for affirming that one has used multiple names, and that on documents both names refer to oneself? It has been recommended to me that I do something like this instead of changing the documents for several living relatives instead of changing the...

Italian Embassy request for Civil Documents Form

Does this mean NY accepts Petitions only if one shows no record for the Naturalization Cert. or can they be presented without dealing with the USCIS?

Comune spelling changes frequently

I have a number of documents that contain the name of the comune Maddaloni, relating to my Italian ancestor. However, this is constantly changing in spelling! So I should request carta libera?

I already sent a request, several weeks ago, to Italy asking for carta legale. Seeing as I did not include €14,62 for the tax, should I assume that it will be ignored and send another request for the carta libera instead?
